Job Summary
The Triage RN provides clinical assessment and guidance to patients via incoming calls transferred from the call center. They will also assist with patient clinical questions/concerns that come in via the phones or in-person from waiting room, patient emergencies in-office, and call EMS/give report if needed. This role uses evidence-based protocols and professional nursing judgment to assess symptoms, determine urgency, and direct patients to the appropriate level of care while supporting safe, efficient clinic operations.

This role uses evidence-based protocols and professional nursing judgment to assess symptoms, determine urgency, and direct patients to the appropriate level of care while supporting safe, efficient clinic operations.
Duties/Responsibilities
- Receive transferred patient calls for clinical triage from call center staff
- Perform comprehensive symptom assessments via telephone
- Utilize established triage protocols to determine level of urgency and disposition
- Identify red-flag symptoms and initiate escalation to provider or emergency services as appropriate
- Work closely with medical assistants, providers, and front desk staff
- Assist in coordinating urgent add‑on visits
- Route patients appropriately (same-day visit, future appointment, urgent care, ED, or self-care)
- Document all assessments, advice, and dispositions accurately in the EHR
- Document assessments and triage decisions accurately in the EHR
- Maintain patient safety, confidentiality, and professional boundaries in all interactions
- Collaborate with call center staff to ensure smooth call transfers and accurate patient information
- Manage high call volumes while maintaining clinical accuracy and professionalism
- Prioritize multiple calls based on clinical urgency
- Participate in quality improvement efforts related to access, triage accuracy, and patient satisfaction
- Office based duties as required
Physical Requirements
- Office-based clinic setting
- Prolonged periods of phone and computer use
- High-volume, task-focused clinical environment
- Ability to move/ lift 30 to 40 pounds to stock supplies and move or support patients
- Use of personal protective equipment (PPE) as required.
Qualifications
- Active RN license (state‑specific requirements apply)
- Minimum 3 years of clinical experience (primary care, urgent care, or triage preferred)
- Strong assessment and critical‑thinking skills
- Excellent communication and multitasking abilities
- Comfortable working in a visible, patient‑facing role
- Proficiency with Athena electronic health records preferred
